Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Splitter added to mop sink faucet and no vacuum breakers present.
Handwash sink not accessible for employee use at all times. Hand sink nearest back door blocked with small trash can. Observed operator correct violations in order to meet inspection standards.
Floor soiled/has accumulation of debris. Floor of walk in cooler had build up of food debris/liquid
Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup inside tall reach in freezer.

Water with a temperature of at least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed hand wash sink at end of cook line only reaching 77F and hand wash sink in front area only reaching 83F.
