Observed in the cook line make table cooler: Sliced tomatoes (48F - Cold Holding); Sliced cheese (48F - Cold Holding); Chopped tomatoes (48F - Cold Holding). Operator the product was placed in there approximately 3hrs ago. Operator placed the product in the standing coolers.
Toxic substance/chemical improperly stored. Observed a chemical spray bottle hangin above containers of ketchup. Operator moved the bottle.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed raw beef at 45F in 4 drawer reach-in cooler. Operator stated they had been in and out of cooler. Operator placed ice on beef to rapidly bring down temperature. Temperature now at 40F.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink in kitchen next to exit. Operator replaced paper towel.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no proof that employees health reporting agreements. Operator stated he had taken folder home that contained health reporting agreements. Provided copies of health reporting agreements to operator. Operator had employees read and sign agreements.
Carbon dioxide/helium tanks not adequately secured. Observed CO2 tanks not secured. Operator put chain around tanks to secure them.

Observed 6 semi dead roaches on sticky trap 2 door reach incooler next to fryers.1 dead in bar area behind soda syrup along wall. Approximately 3 dead in dry storage area, 1 on shelf and 2 beneath wire rack. 1 dead roaches in women's bathroom. 1 dead roaches along wall in corner next to dry storage area.
