Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed blue cheese crumbles 57F and homemade blue cheese dressing 51F in walk in cooler at 1:45. Per manager door was held open while stocking other food items at 10:30-12:30. Manager iced food items during inspection and corrected to blue cheese crumbles 47F and homemade blue cheese dressing 51F. See stop sale.

Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed walk in cooler ambient temperature 60F at 1:45-3:00. Manager called maintenance for repair during inspection.

Dead roaches on premises. Observed 2 dead roaches outside of men's bathroom on floor and 1 dead roach on floor of dry storage area. Manager swept up and sanitized the areas.
