Reach-in cooler interior/shelves have accumulation of soil residues. Observed interior bottom of reach in freezer soiled with fries.

Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee handle raw chicken, then proceed to handle clean food equipment without changing glove or washing hands. Manager reviewed proper hand washing procedures with employees. Employee washed hands.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup in since reach in freezer at cookline. Per Operator, freezer put in defrost mode.
