Observed 19 dead roaches in the following locations: 1 in unused dining room under tables, 1 under discarded fryer in unused dining area, 1 next to unused soda machine in unused dining room, 2 by walk-in cooler on floor, 1 between reach-in coolers on floor in prep/ ware washing area, 5 behind reach-in coolers in prep/ ware washing area, and 8 in office in back of establishment.

Observed 19 dead roaches in the following locations: 1 in unused dining room under tables, 1 under discarded fryer in unused dining area, 1 next to unused soda machine in unused dining room, 2 by walk-in cooler on floor, 1 between reach-in coolers on floor in prep/ ware washing area, 5 behind reach-in coolers in prep/ ware washing area, and 8 in office in back of establishment.
Exterior door has a gap at the threshold that opens to the outside. Gaps in threshold of back door, as well as a hole from a missing door knob that has not been filled in.
Floor soiled/has accumulation of debris. Floor in mop sink area across from office and next to back door, and behind grill are soiled with heavy grease build-up,

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Bottom of prep shelf of table between ware washing area and kitchen covered with flour.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach-in cooler on make line: Tzatziki at 56f for 2 hours, burgers 58f for 2 hours. Operator turned down cooler and placed ice bucket in cooler to help cool down unit.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling soiled with dust in kitchen.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Reach-in cooler on make line is in disrepair, evident by duct tape holding gaskets onto doors. Reach-in cooler is not holding food cold, ambient air in cooler is 54f. Operator filled container with ice to and placed inside until to help keep unit cold.
Dead roaches on premises. Observed 13 dead roaches in the following locations: nine under reach-in coolers near 3 compartment sink, two under 3 compartment sink, one under prep table near wall near 3 compartment sink, and one under prep table on make line. Operator began cleaning and sanitizing areas during inspection.
Exterior door has a gap at the threshold that opens to the outside. Gap along threshold and hole in door at back door, front entrance has gap along top threshold.
Time/temperature control for safety food thawed in an improper manner. Frozen gizzards thawing in ambient air. Operator placed them Nader cold running water using inspection.

Dead roaches on premises. Observed six dead roaches on floor in front dry storage location.
Floor soiled/has accumulation of debris. Observed excessive chicken juice and debris on floor in walk in cooler.
Food stored on floor. Observed multiple food items stored on floor in front dry storage location.

Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Upon inspection observed employee water sitting on top of cutting board in main make line in kitchen.
Time/temperature control for safety food thawed in an improper manner. Upon inspection observed frozen chicken bags sitting on counter, operator stated they were thawing. Operator was instructed to put chicken wings under cold running water.
Wall soiled with accumulated grease, food debris, and/or dust. Upon inspection observed walk-in walls soiled with mold like substance.

Raw animal food stored over/not properly separated from ready-to-eat food. Observed frozen shrimp over open shredded cheese in each in cooler on make line. Employee moved switched cheese and shrimp on rack.
Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed chicken wings in walk in cooler at 45F held 24hrs.

Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed 2 dead flying insects in the following areas: 1 in dining room used as storage area and 1 in back kitchen in unused container on storage rack. Operator cleaned up flying insects at time of inspection.
Dead roaches on premises. Observed 1 dead roach in office room. Operator cleaned up dead roach at time of inspection.
Exterior door has a gap at the threshold that opens to the outside. Observed rear kitchen door with gap at floor and side. Door is not closed completely.
Food not stored at least 6 inches off of the floor. Observed cooking oil and sauces stored on floor in dining room used as storage area.
Objectionable odors in bathroom or other areas of the establishment. Observed strong odor coming from office room.
