Observed various raw meats stored using a reduced oxygen method which does not comply with the procedures stated in the Process Waiver or HACCP plan approved by the Division of Hotels and Restaurants Establishment was approved for "vacuum packing fish 10k bags" only. Advised manager to reapply for HACCP plan to package raw meat. Establishment must cease practice until approved. Stop sale issued for All meats with date exceeding 48 hrs.
Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw shell eggs and raw salmon being stored over various ready to eat foods in walk in cooler.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed the following food temperatures greater than 41°F; raw salmon (61F - Cold Holding); raw chicken (60F - Cold Holding) All were located in reach in cooler in kitchen area( shawarma station). Cooler had an ambient temperature of 54°F. Per manager all foods had only been in cooler for about an hour. Manager removed all foods to walk in cooler. Maintenance arrived during time of inspection to repair.
No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. Manager arrived during time of inspection.

Observed reach in cooler near kitchen entrance with ambient temperature of 47°F and cooler under kitchen equipment with ambient temperature of 54°F. Per manager all foods had only been in cooler for about an hour. Maintenance arrived during time of inspection to repair.
