Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishmachine tested at 0ppm chlorine. Operator adjusted machine and retested at 50ppm chlorine.
Cardboard used to line food-contact shelves. Cardboard used to line shelving in walk-in cooler. Operator removed cardboard.

Operator marked all menus with an asterisks on the tuna tataki during inspection. Consumer advisory reminder already indicated on the menu.
Raw fruits/vegetables not washed prior to preparation. Cut avocado with sticker still attached on counter at sushi area. Operator voluntarily discarded avocado.
Wet mop not stored in a manner to allow the mop to dry. Wet mop stored outside back exit not inverted. Operator stored mop properly.
