In bottom of flip top reach in cooler at front counter, container of milk opened yesterday morning not date marked. Employee added date to container.
Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine/sanitizer bucket. Establishment has Quat test strips but use bleach for three compartment sink and sanitizer buckets.
No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Operator states that all items at front counter are held on time but can not provide any written procedures. Inspector printed DBPR form 5022-100 for operator and operator filled out during inspection.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine soiled with mold-like substance. Employee cleaned interior of ice machine during inspection.
Bowl or other container with no handle used to dispense food. In containers of m&ms Reese's and rainbow sprinkles on bottom shelf of front counter, small plastic cups used as scoopers. Employee removed cups from products and discarded.
Equipment in poor repair. Gaskets in reach in cooler in kitchen torn.
Floor, wall and/or ceiling soiled/has accumulation of debris. Wall behind yogurt machines in kitchen soiled/stained with food debris.
Food stored on floor. White tub with bags of boba tea powder stored on floor behind front counter. Also, in back kitchen across from 3 compartment sink multiple containers of sugarcane syrup stored in floor. Employee moved all items off of floor.
Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Light shield missing from light in kitchen near door.
Old labels stuck to food containers after cleaning. Multiple old labels on containers being used for other products at front counter and reach in cooler in back kitchen.
Twenty (20) foot-candles of light at least 30 inches from floor not provided in handwashing, warewashing, or equipment and utensil storage areas. Light above 3 compartment sink not working.
Wet wiping cloth not stored in sanitizing solution between uses. Two wet wiping clothes stored on front counter in between uses. Employee put both clothes in sanitizer bucket.
Wiping cloth chlorine sanitizing solution not at proper minimum strength. At front counter, Sanitizer Bucket (Chlorine 0ppm). Employee added bleach to bucket and rechecked at 100ppm.

Establishment has tapioca pearls on time at front line but none of the pearls are time marked. They were pulled out at 11 am this morning. Manager time marked all pans.

In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. At front counter on bottom shelf, handle of scoop in peanuts touching product. Employee removed handle of scoop from product.
Light not functioning. Above ware washing area in back of kitchen light does not work.
Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. At entrance of kitchen area, light has no cover or shield.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Fan covers in reach in cooler in kitchen area have build up of dust.
Old labels stuck to food containers after cleaning. Multiple containers have old labels still on them while being used for a different product. In reach in cooler in kitchen area, container with label m&ms on lid contains chocolate chips.
Wet wiping cloth not stored in sanitizing solution between uses. On front line, wet wiping cloth on lid of reach in cooler. Employee removed and stored back in sanitizer bucket.
