Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine with a black/green mold-like substance. Operator cleaned ice machine.
In-use tongs stored on equipment door handle between uses. Observed tongs on oven door handle. Operator moved for proper storage.

Observed dish machine testing at 0ppm after running three times. Operator set up manual sanitizer and called technician.
Spray bottle containing toxic substance not labeled. Observed spray bottle of chemical not labeled with common name. Operator provided labels.
Equipment and utensils not properly air-dried - wet nesting. Observed wet nesting of metal pans and plastic containers above three compartment sink. Operator separated to dry properly.
