In flip top reach in cooler on cook line, shredded mozzarella (51F - Cold Holding); cut tomatoes (52F - Cold Holding); hotdogs (50F - Cold Holding); cut lettuce (52F - Cold Holding). Operator stated all items were put in reach in cooler approximately 3 hours prior. Operator moved all Time/Temperature control for safety foods to reach in cooler.
Toxic substance/chemical improperly stored. Spray bottle of degreaser hanging on shelf of metal rack on cook line used to store single service to go boxes and gloves. Operator moved spray bottle to shelf under 3 compartment sink used for chemicals.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior deflector plate of ice machine soiled with black mold-like substance.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Flip top reach in cooler on cook line ambient air thermometer reading 55F, actual ambient air temperature at 53F. Do not store Time/Temperature control for safety foods in cooler till fixed.

Dead roaches on premises. Observed approximately thirty (30) dead roaches in kitchen area behind refrigeration equipment and ice machine. Employee began removing all dead roaches.
