Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Observed paper towels at hand wash sink in warewashing unable to dispense properly. Operator started to change out dispenser during this inspection.
Soap dispenser at handwash sink not working/unable to dispense soap. Observed soap dispenser at hand wash sink on front service line and in prep area unable to dispense soap. Operator worked on it to get it dispenser.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee personal belonging on cutting board on cook line. Employee removed cell phone to properly store.
Floor tiles missing and/or in disrepair. Observed diamond plate floor in walk in freezer in disrepair/ buckled up at seam.

Also, gasket on reach-in cooler below make station soiled with an accumulation of debris. Also, shelf and bracket of shelf above ice cream machine at front service line soiled.

Employee rubbed hands together for less than 10-15 seconds while washing hands. Employee in kitchen on make line washed hands for approximately three (3) seconds. Manager coached employee on proper hand washing procedures. Employee washed hands correctly.
Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Cook in kitchen touched bare midsection of body while adjusting shirt/pants with gloved hands and immediately engaged in food preparation without removing gloves and washing hands. Manager coached employee on proper hand washing procedures. Employee washed hands correctly.

Employee has not received adequate training related to their assigned duties as evidenced by the inability to answer basic food safety questions. Cook working fryers did not know minimum cooking temperature for chicken. Inspector coached employee on temperatures.

Operator stated the chicken was taken out at lunch rush and was out for about 20 minutes. Operator placed raw chicken in walk-in-cooler to chill.
