Person in charge states items have been out of temperature for less than 4 hours, moved to walk in cooler. In right side drawers at cookline chicken 53F. Person in charge moved to walk in cooler. In breading station far left chicken filets at 45F. In walk in cooler chicken on speed rack 50-51F Person in charge states all items out of temperature for less than 4 hours, moved to walk in cooler/ freezer to rapidly chill.
Food-contact surface soiled with food debris, mold-like substance or slime. Deflector plate at ice machine in dining room with debris buildup.
Proof of required state approved employee training not available for some employees. One employee working at establishment for 13 years with no proof of training.
Dead roaches on premises. Observed two dead roaches on the floor in back storage area by water heater.
