Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Located in back of house near 3 compartment sink. Manager notified that additional vacuum breaker is needed due to splitter and shown example of vacuum breaker.

Had multiple cards not fully filled out and missing training dated as well.
