Handwash sink used for purposes other than handwashing. Observed equipment inside hand-wash sink next to prep sink. Manager removed equipment from hand wash sink during inspection.
Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Observed paper towel dispensers inoperable at three hand wash sinks: next to prep sink, next to salad station, and next to reach in freezer by cook line. Manager stored available paper towels during inspection.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed accumulation of dust on ceiling in walk in cooler and above cook line.
Exterior door is left open to the outside. Manager closed door during inspection.
