Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee at cookline adjusted hat with bare hands, then put on gloves. Manager stopped employee and had them wash their hands and put on new gloves before continuing to work at cookline.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Backflow prevention device missing at hose connection on wall inside dumpster enclosure.
Records/documents for required employee training do not contain all of the required information. Two Florida Restaurant and Lodging Association food handler certificates missing date of training and CFPM number. Manager finished filling out the certificates during this inspection.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling tiles near back door of kitchen soiled with dust.
A return visit. The problems from the previous inspection had been dealt with.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On shelf inside walk-in cooler: chicken (49F - Cold Holding). Manager stated the chicken was moved to the walk-in cooler from a cooler on the cookline approximately 2 hours prior and stated it may have sat on the counter too long. Inspector had manager place the chicken into the freezer to rapidly cool to 41F. chicken (49F 12:00, 36F 12:30 - Cooling)
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In warming unit at front counter: chicken (93F - Hot Holding). Manager stated the chicken has been in warmer since this morning and voluntarily discarded the chicken during this inspection. Manager stated the chicken is held using temperature for safety and time for quality.
Toxic substance/chemical improperly stored. Two condiment cups containing dish soap stored on shelf directly above box containing single-use bowls in kitchen near three-compartment sink. Employee placed dish soap on the sink during his inspection.
Handwash sink used for purposes other than handwashing. Ice dumped into handwashing sink at front counter.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee backpack and jacket stored on shelf in storage area directly above boxed fountain sodas. Manager removed both from shelf during this inspection.
Garbage on the ground and/or pad around dumpster. Trash scattered on ground around dumpster inside enclosure.
Old labels stuck to food containers after cleaning. Remnants of old labels stuck to side of two pans on clean dish rack.
Single-service articles improperly stored. Box containing single-use gloves stored directly on floor in office. Manager removed gloves from floor during this inspection.
Unclean building components, attachments or fixtures. 1. Several ceiling tiles above cookline soiled with dust. 2. Ceiling tile in back of kitchen near back door soiled with dust.
