Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed five employee training certificates, manager stated there are 15 employees.
Old food stuck to clean dishware/utensils. Pans stored on shelf across from three compartment sink.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. "Y" connector at mop sink is missing vacuum breaker on the non-chemical side of the "Y" connector. Employee placed vacuum breaker on no-chemical side attached to hose.
Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tiles missing around walk-in-freezer
Duct tape used to repair nonfood-contact surface. Duct tape used in several places above and around walk-in-cooler and on handwash sink soap dispenser
Floor tiles missing and/or in disrepair. Missing floor tiles around walk-in-freezer with standing water where tiles are missing and around ice machine.
A return visit. The problems from the previous inspection had been dealt with.

Ceiling tile missing. Multiple around cook line, ware washing area and storage area, also multiple tiles cracked and with standing dirty water.
Current Hotel and Restaurant license not displayed. June 1, 2022. Manager request current license, printed and posted during inspection.
Dead roaches on premises. Observed one dead roach on floor near front fryer. Also observed one dead roach on floor near handwash sink at the three compartment sink area, manager cleaned.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee jacket on top of peach syrup box, manager moved.
