Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Splitter at mop sink, backflow preventer located on faucet before splitter. Vacuum breaker needs to be located at end of splitter on non chemical dispenser side.
Handwash sink used for purposes other than handwashing. Handwash sink behind front registers, Blue towel in sink. Handwash sink at end of cook line, broom and dustpan blocking sink. Operator removed all items.
