Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee on cook line touching hair while adjusting hat with gloved hands, then continued to handle clean utensils and make food orders without changing gloves and washing hands between tasks.
Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. In use triple sink reading 0ppm, manager remade to 200ppm
Handwash sink not accessible for employee use at all times. Large brush in hand wash sink on cook line by fryers, brush moved.
