Time/temperature control for safety food required to be date marked is not date marked and time of opening/preparation cannot be determined. See stop sale. Open jug of milk inside reach-in cooler without date marked manager stated the milk was opened approximately one week prior, but was unable to determine which day.
Toxic substance/chemical improperly stored. Can of plumbing cement stored directly on top of ice machine.
Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Open jug of milk inside reach-in cooler without date marked manager stated the milk was opened approximately one week prior, but was unable to determine which day. Stop sale issued.
No proof of required state approved employee training provided for any employees. No proof of food handler training available for any employees. One employee working in kitchen and handling food who has been employed more than 60 days.
No soap provided at handwash sink. No soap at handwash sink near ice machine.
Bowl or other container with no handle used to dispense food. Single-use cup without handle stored inside container of paprika on dry storage shelf.
Cleaned and sanitized equipment or utensils not properly stored. Several boxes containing single-use food containers stored directly on floor in dry storage closet. Manager removed all items from the floor during this inspection.
Dead roaches on premises. One dead roach on floor in kitchen, near dry storage shelf. Manager swept the area during this inspection.
Food being prepared outside. Partially covered pot of uncooked rice soaking in water outside on table. Inspector had manager move the rice back into the kitchen.
Food stored on floor. Several bags containing rice, onions, and containers of sugar and oil stored directly on floor throughout kitchen.
Outer openings not protected during operation and vermin and/or environmental cross contamination present. Door to kitchen and two windows without screens left open. Two flies and one dead roach present in kitchen.
Unclean building components, attachments or fixtures. Floor in storage closet soiled with dirt and/or leaves.
A return visit. The problems from the previous inspection had been dealt with.
