Employee not wearing gloves wiped face and arms with cloth, placed cloth on top of flip-top make table cooler, then opened flip-top make table without washing hands and putting on clean gloves. Same employee later scratched his face then handled cooking equipment without washing hands and putting on clean gloves.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Raw shelled eggs at 48F stored in flip-top make table cooler. Raw shelled eggs were above chill line in container. Manager advised that eggs had been placed there 30 minutes earlier. Manager removed eggs above chill line and place in reach in cooler
